I authorize ON CALL to contact the persons and organizations I have provided or listed on my resume to confirm that the information is true and correct and to further evaluate my qualifications for employment. I authorize all previous employers, educational institutions, consumer reporting agencies and other persons or entities having information about me to provide such information to ON CALL or any entity authorized to obtain information for ON CALL without giving me prior notice of such disclosure. I fully release ON CALL, its employees, officers, agents, successors and assigns, and all other parties involved in investigating my background, including but not limited to investigators, credit agencies and those companies or individuals who provide information to ON CALL concerning me, from any claims or actions for any liability whatsoever related to the process or results of the background investigation. I understand that any offer of employment is contingent upon the outcome of a background check.

PHYSICAL EXAMINATION AND DRUG AND/OR ALCOHOL TESTING: If I am offered an assignment to work for a client of ON CALL that requires a pre-employment physical examination, which may include drug and/or alcohol testing, I understand that such job assignment offer is contingent upon successfully passing the physical examination. I also understand that by my acceptance of the contingent job assignment offer, I agree to undergo such physical examination, including drug and/or alcohol testing, and consent to the release of the results to the Client Company and ON CALL. I understand ON CALL maintains a Substance Abuse and Testing policy that provides for drug and alcohol testing when there is a reasonable suspicion of drug or alcohol use. I understand and agree that any offer of employment with ON CALL is conditioned upon my compliance with its policies and directives.

ACKNOWLEGMENT OF MUTUAL ARBITRATION AGREEMENT: As a condition of employment, I acknowledge I will be required to separately execute a Mutual Arbitration Agreement with ON CALL, for the resolution of any potential disputes arising from my application, employment, job assignments, or termination of job assignments or employment, according to its terms.

AT-WILL EMPLOYMENT: I understand and agree that if I am employed by ON CALL, my employment will have no specified term and may be terminated at any time, with or without cause or notice, by either myself or ON CALL. In addition, if I am hired, ON CALL will have the discretion to impose any form of discipline it deems appropriate or to alter my position or assignment at will. I understand that no representative of ON CALL may enter into any contrary agreement unless it is done specifically, in writing, and is signed by the CEO of ON CALL or their authorized representative.

ELECTRONIC TRANSACTION: acknowledge that I have received and reviewed this Employment Application by electronic means, and that this electronic version of the Employment Application is an effective representation of my responses to the information requested, including this Certification. By completing this form, checking the "I Accept" box, and submitting this information to On Call Staffing Solutions, Inc. (“ON CALL”), I agree to conduct transactions electronically with ON CALL, using a computer or other electronic device to receive, view, electronically sign, and return records, including this Employment Application. I also acknowledge and agree that my electronic signatures will consist of clicking on electronic buttons and/or checking electronic boxes where indicated. In addition, I consent to ON CALL’s provision of records to me electronically by emailing them to me at my email address or by any other electronic means that we have mutually agreed upon. I further acknowledge and agree that such electronic means may include electronic records as attachments or as embedded links to a website that ON CALL operates and controls. I further acknowledge that this electronic Employment Application is in a form that is capable of being retained and accurately reproduced for later reference.
